Memorial Donation

Monetary gifts may be given for the purchase of books or to the general fund in memory of family members and friends or in honor of a birthday, anniversary, or other special occasion.  Each book is marked with a gift plate and a card is placed in a permanent file.  Please copy, print out, complete, and mail the following form:
